Description

Description (Service) Provide residential care in a holistic manner to individuals who have special needs. Programs are designed to:
  • provide a change environment that's both therapeutically and socio-emotionally supportive
  • ensure the safety of each individual in residence, also with the use of a program of external controls (behavior modification)
  • make the environment conducive to the individual resident's functional ability in order to help eliminate high risks behavior
  • provide a multifaceted assessment process in order to holistically identify resident's levels of functioning, programming and needs, socio-emotional challenges and risks
  • help residents realize their individual potential through the acquisition of adaptive social skills including basic self care, communication, community integration, and skills development
  • ensure that each resident's individualized plan of care is reflective of goals consistent with the resident's level of functioning and personal goals
  • involve the resident's family or person placing the resident in all aspects of the resident's care in order to provide the best quality care, instill a sense of belonging for resident, and maintain family responsibility where applicable
