| Eligibility | Programs are for residents of Huron-Kinloss and Kincardine and self-referral is accepted for most services. Cardiac Rehab requires a Physician or Nurse Practitioner referral. Memory Clinic only accepts referrals from Kincardine Physicians and Nurse Practitioners. |
