Description & Services

Description (Service) Provides diabetes management support for anyone at risk of or living with diabetes looking to learn about how to prevent their risk for developing diabetes, delay the progression of diabetes or to improve their current blood sugar control.

Visits to this clinic include education in areas such as:
  • have your blood pressure checked
  • understand how to check your blood sugar to see patterns
  • talk about healthy eating and exercise
  • discuss how your medication and insulin work
  • have a foot assessment completed
  • learn how to set goals
  • review your lab tests (A1c, cholesterol, kidney function)
  • discuss your feelings and emotions about having diabetes
Hours Mon-Fri 8 am-4 pm

Eligibility
  • Individuals 18 years or older at risk of or living with diabetes
  • Registered patient of a doctor at the North Simcoe Family Health Team
