Description
| Description (Service) | Part of a provincial team created to support youth, up to age 25, who may have difficulty connecting to services and will benefit from intensive one-on-one support in the community. Provide clinical counselling to youth while works with them and their families to better navigate and connect to services and opportunities. Delivered in partnership with YES (Shelter for Youth and Families) In person and virtual services offered on a case by case as needed basis. |
