Description (Service)

Services include:

supporting students to transition from school to adult life
assisting students to direct and develop an individualized transition plan in conjunction with people most important to the students
assisting students to explore and identify future community options available after high school, including post-secondary education, employment, volunteering, and social and recreational opportunities
securing and leveraging available resources, supports, and funding
liaising with the school system and other youth services and organizations
